Patient Forms, Map and Directions to Honolulu Office

Your first visit to Fujioka Dental Group, Inc. establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by completing the Online Medical and Dental History forms (see below) in advance of your appointment.

Mission Statement
Our practice is working together to realize a shared vision of honest, uncompromising excellence in dentistry.

To fulfill this mission, we are committed to:

  • Listening to those we are privileged to serve.
  • Earning the trust and respect of patients, profession and community.
  • Exceeding your expectations.
  • Ensuring a creative, challenging and compassionate professional environment.
  • Striving for continuous improvement at all levels.

Comprehensive Oral Examination and Risk Assessment
Your initial oral examination includes a visual examination, charting, periodontal probing, oral cancer screening, functional analysis, diagnosis and treatment recommendations. Understanding that a wellness model is a better way to approach dental health care than a repairative model, we evaluate your mouth in four main areas- Teeth, Gums and Bone, Jaw Joints and Function, and Esthetics.  We then assess your risk factors in each of those four areas to arrive at a treatment plan to reduce your risk for future problems and improve your prognosis.  We will also take x-rays, which includes the panoramic x-ray for proper diagnosis of the anterior (front) and posterior (back) teeth as well as the bite-wing x-ray series for proper diagnosis of proximal decay of posterior teeth.  The key is to make an accurate diagnosis of your condition so that appropriate care can follow.
